***IMPORTANT - Permission Form (Tryouts)***
If attending a tryout, a Tryout Permission Form must be completed and submitted by a player that has minor eligibility remaining from his home centre, and players that are members of a Club system. Junior players who are trying out for a higher category team within the OHA, do not require a Permission form. The form must be obtained directly from a team representative; it is not available for download.
